Campus policy requires all labs to have emergency contact information posted on
the lab door.
- If you do not currently have emergency numbers posted on the lab door, please
use this form. If you have a list posted, please use this form next time a
revision is necessary.
- Place completed form in EHS provided sleeve (call 4-2850 for sleeve) and attach
to exterior of lab door.
- Each lab needs a separate form.
- All listed contacts should have knowledge of the lab contents and the work
currently in progress.
- A home or cell phone number is preferred for the phone contact information
so that we can reach in the event of an after hours or weekend emergency.
- This information should be updated by the lab on a regular basis.
